The real survey, in its own words
The first page asks for three things from a receipt and nothing else: the store number, the date and the time. No name, no email, no card and no login before the questions, and no prize is named on the page read. A Spanish version is offered on the same page.
The store number
Every one of the 452 restaurant numbers read for this site has six digits, and 452 of them begin with a zero, which is why the survey says to enter every digit including zeroes; the lowest read was 000080 and the highest 043120. The number is at the top of the receipt, in the survey’s words, and it is the same number the brand’s ordering menu uses for the restaurant, so a store number on this site’s state pages is the survey’s store number. The contact page places the order number on the same receipt: “Your order number can be found at the top of your printed receipt, below the date.”

A search for the survey turns up pages that are not tellthebell.com: guides that ask for an email before showing the questions, pages that name a prize the real page does not, and forms that ask for payment details. The real page asks for a receipt’s three fields and no payment information at all; a page that asks for a card number, a login before the questions or a fee is not the survey, whatever it is called. This site links the real page in the sources and reproduces none of its questions.
Feedback that is not a survey
A complaint about an order does not go to the survey: the customer service page has the form and the line, and a partner delivery order goes to the partner. A missed reward goes to the app’s receipt scan, in the brand’s words: “Points should appear within 48 hours.”
What this site does with reviews
Nothing. TB Menus publishes no reviews, ratings or testimonials, its own or anyone’s; it prints the brand’s figures and its own reads, and the editorial policy sets that out. A page that shows star ratings for a restaurant is showing someone else’s, and the FTC’s 2024 rule on fake reviews is in the sources for the reader who wants the line drawn.
